Skilled Nursing

Experienced Registered Nurses, assisted by Licensed Vocational Nurses, implement a comprehensive, individualized plan of care in conjunction with the client’s physician. Observation and assessment of a patient’s condition with acute or chronic illness, teaching and training, catheter care, wound care, nutritional needs, oxygen, rehabilitative care to include bowel and bladder training and medication management.

Licensed Physical Therapists

Licensed Physical Therapists assist patients rehabilitate and improve gait, transfers, range of motion, provide therapeutic intervention for pain control, instruct caregivers in methods of assisting patients in transfers and gait, enhance the functional abilities of the patient or increase the ease of caring for the patient.

Licensed Occupational Therapists

Licensed Occupational Therapists help patients regain their ability to perform bathing, dressing toileting, cooking and eating. Assist patients who require instruction in one handed techniques, impaired cognition, perception or awareness of body parts. Instruction provided for joint protection, energy conservation techniques, and may recommend adaptive equipment to enhance their functional ability.

Licensed Speech Therapists

Licensed Speech Therapists assist patients with decreased ability to express and/or receive communication, with impaired cognition and/or memory, ability to phonate, and dysphagia with swallowing instructions.

Adult Nursing services

TEXMED Adult Nursing services help adults of all ages with ongoing care needs related to chronic illness, injury, or disability. These needs range from the management of conditions like diabetes to complex, high-tech tracheostomy and ventilator nursing care.

These services are provided by registered nurses (RNs) and licensed practical nurses (LPNs) primarily on an ongoing shift (two-hour or more) basis, and are available 24 hours a day, 7 days a week. This care is also sometimes referred to as private duty or adult skilled nursing.

All TEXMED Nurses are home health care specialists. Our RNs and LPNs are highly experienced and knowledgeable professionals who are committed to providing quality, compassionate care to their clients and peace of mind for their families.

Home Health

TEXMED Home Health services help adults and seniors who are recovering after a hospital or facility stay, or need additional support to remain safely at home and avoid unnecessary hospitalization. These Medicare-certified services may include short-term nursing, rehabilitative, therapeutic, and assistive home health care.

This care is provided by registered nurses (RNs), physical therapists (PTs), occupational therapists (OTs), speech language pathologists (SLPs), home health aides (HHAs) and medical social workers (MSWs) as a limited number of up to one hour visits, primarily through the Medicare Home Health benefit.

What counties do you cover?

Do I have to get a doctor’s referral?

While a Homehealth client can be referred by anyone, doctor, hospital discharge planner, self, family, etc. a doctor must sign for services. Tex Med Home Health works very hard to work as the Doctor’s eyes and ears in the patients’ home and most doctors appreciate the value of Home Healthcare.

 

Who chooses which Home Healthcare?

The ultimate decision belongs to the patient. In the same manner you can choose which doctor you go to, you also have the choice of Home Healthcare agencies.

 

Who pays for Tex Med Home Health’s services?

If you meet the criteria for Medicare Home Health Care, the services are free to the patient.

 

Who is eligible for Medicare Home Health Care?

  • Doctor must agree that you need care in your home
  • You must need at least one of the following: intermittent skilled nursing care, Physical Occupational or Speech therapy services and
  • You must be homebound. This means that you are normally unable to leave home on your own. Being homebound means that leaving home is a major effort. When you leave home it must be to attend Adult Day Care, or infrequent, for a short time or to get medical care.
  • The Home Health Agency must be approved by the Medicare program.
